For Social Workers & Placing Authorities

Make a Referral

We work closely with social workers, local authorities, and placing teams to ensure every placement is the right fit. Our process is straightforward, transparent, and always led by the needs of the young person.

How Our Referral Process Works

We understand that finding the right placement quickly and confidently is critical. Our team responds promptly to all referrals and will guide you through every step of the process — from initial enquiry through to placement and beyond.

01

Initial Enquiry

Contact our referral team by phone or email to discuss the young person's needs, our current capacity, and whether we may be a suitable match. We aim to respond to all enquiries within 2 hours during working hours.

You can reach us on 0208 092 0739 or email [email protected]. An emergency line is available 24/7 for urgent placements.

02

Referral Information

If there is potential for a match, we will ask you to share a referral pack including the young person's care history, current needs, risk assessments, and any relevant reports.

All information is handled with strict confidentiality in line with GDPR and data protection legislation.

03

Suitability Assessment

Our registered manager and responsible individual will review the referral information and assess whether we can meet the young person's needs safely and effectively.

We will consider the young person's needs alongside the needs of existing residents to ensure a safe and positive environment for all.

04

Pre-Placement Visit

Where possible, we encourage a pre-placement visit so the young person can see the home, meet key staff, and feel comfortable before moving in.

We can also arrange virtual tours or introductory calls if an in-person visit is not immediately possible.

05

Placement Agreement

Once a placement is agreed, we will work with you to complete all necessary paperwork, including the placement agreement, care plan, and risk management documentation.

We aim to have all documentation in place before or on the day of placement to ensure a smooth transition for the young person.

06

Ongoing Review & Support

We maintain regular communication with placing authorities, attend reviews, and provide written reports as required. Our door is always open to discuss any aspect of a young person's placement.

We are committed to working in partnership with social workers, families, and all professionals involved in a young person's care.
